Meeting Sponsorship Agreement This agreement between the Opticians Alliance of New York Inc, and _____________________________, to sponsor a meeting to be held on ________________________at the Millerridge Inn in Jericho, NY. The Opticians Alliance of New York (hereinafter called OANY) will provide to the sponsor 2 or more tables as needed for the sponsor to be able to set up there displays and signage. Further OANY will provide to the sponsor 2 free dinners for their representatives. OANY will advertise the meeting in their newsletter and on their website at no additional cost to the sponsor, the type of advertising shall be at the discretion of OANY. In addition if the sponsor wishes to have more than two representatives at the meeting for dinner then the additional cost shall be assumed by the sponsor. The sponsor shall, where necessary furnish their own audio visual equipment. If the sponsor does not have said equipment then OANY shall rent the equipment , and the sponsor shall be responsible for said rental. The sponsor if also providing continuing education must forward to OANY the resume of the speaker , his course outline and any power point presentation , for submission to our continuing education provider. In accordance with the terms outlined above the sponsor shall pay to OANY the sum of $750.00 plus the additional dinner and rental fees , one Month prior to the meeting date listed above.
